COVID Vaccine submission
United Way Worldwide will be partnering with Attend Safe for the upcoming United in Purpose event, to provide secure and HIPAA compliant COVID-19 vaccine verifications. All United in Purpose attendees are required to provide proof of COVID-19 vaccination or test negative to a rapid COVID-19 test administered by the licensed Attend Safe team onsite at United in Purpose. You will find more information about both options below. For attendees without proof of COVID-19 vaccination or for those who prefer to provide a negative rapid COVID-19 test, onsite testing will be provided on Wednesday, May 17 from 7:30 am to 5:30 pm.
If you are a non-U.S. citizen who is a nonimmigrant (not a U.S. citizen, U.S. national, lawful permanent resident, or traveling to the United States on an immigrant visa), the United States currently requires travelers to show proof of being fully vaccinated against COVID-19 with the primary series of an accepted COVID-19 vaccine to travel to the United States by plane. COVID-19 Vaccine Verification Submission:
As part of the United in Purpose Health & Safety Requirements, you will be using the Attend Safe web-based application for the submission of your vaccination card/records, for validation (HIPAA secure verification). Please note: this is not a "downloadable app" from Apple or IoS. The web-based application is only accessible through the internet. We encourage you to access the web-based application below and submit your vaccination card/records for verification by May 10.
Follow the instructions below for submitting COVID vaccination information as required for attendance:
- Scan the QR Code provided (access via the camera on a smartphone or tablet) OR go to: www.attendsafe.app/UnitedWay/May2023
- Follow the prompts.
- Enter the information requested (including COVID vaccination card/records).
- Select “upload the image” of your COVID-19 vaccine card/documents.
- Once your COVID-19 vaccine card/documents are submitted and verified, you will receive an email confirming your verification.
Acceptable Vaccination Documents:
- Actual photo of your CDC issued vaccination card clearly showing your full name, vaccination type received, date of shot(s) and administering facility/clinician.
- Medical records (EMR) including your full name, vaccination type received, date of shot(s) and administering facility/clinician.
If additional information is required to complete your verification, the Attend Safe Verifications Team will contact you via the email and/or cell phone number provided during your submission. If you do receive a follow-up request for additional information, please make sure to respond in a timely fashion, so your entry into the event is not delayed upon your arrival. We suggest that you save and/or print and bring with you, the email verification confirmation for entry to the event.
On-site COVID-19 Test Verification:
If you plan to receive an on-site clinically administered COVID-19 rapid antigen test, Attend Safe’s team will be on-site administering the tests and confirming the negative results for your attendance at the event. On-site testing will be available on Wednesday, May 17, 2023, starting at 7:30 a.m. in Room 330. Please plan approximately 30 minutes for the testing process.
Once you’ve received a negative test result, Attend Safe will provide you with proof of your negative test. Then you can proceed to the registration area to get your badge/credential. Without your Attend Safe clearance, you will not be permitted into the event. Please visit the testing room BEFORE heading to registration.
