COVID VACCINE SUBMISSION

United Way Worldwide will be partnering with Attend Safe for the upcoming Advocacy Forum & Capitol Hill Day event, to provide secure and HIPAA compliant COVID-19 vaccine verifications. 

All Advocacy Forum attendees are required to submit their proof of vaccination by June 13, or you will not be able to gain entrance to the event. To proceed with providing your proof of Covid-19 vaccination, follow the instructions below. Once submitted, your documents will be reviewed and verified for authenticity and confirmation of event attendance. A separate submission must be done for each individual/attendee.

If you have circumstances which prevent you from submitting your proof of vaccination, please contact events@unitedway.org by June 13.

During the event, we encourage following recommended health and safety protocols, including regular handwashing, maintaining social distance, avoiding poorly ventilated spaces, and self-isolating if you feel ill or test positive for COVID.  Masks are not required but are encouraged.  Should you become ill or test positive for COVID-19, we request that attendees remove themselves from participation and notify events@unitedway.org. United Way reserves the right to deny entry to or remove anyone at the event presenting with flu-like or COVID-19-related symptoms. Refusal to abide by United Way Worldwide’s health safety related procedures may result in removal from the program and cancellation of registration without refund.    

United Way Worldwide has put in place various preventative measures to address concerns about the spread of COVID-19. Nevertheless, we cannot guarantee that attendees will not be exposed to COVID-19 while attending the conference. Each attendee assumes the risks associated with attendance at an in-person event.    


COVID-19 Vaccine Verification Submission:
As part of the Advocacy Forum & Capitol Hill Day Health & Safety Requirements, you will be using the Attend Safe web-based application for the submission of your vaccination card/records, for validation (HIPAA secure verification). Please note: this is not a "downloadable app" from Apple or IoS. The web-based application is only accessible through the internet. We encourage you to access the web-based application below and submit your vaccination card/records for verification by June 13.

Follow the instructions below for submitting COVID vaccination information as required for attendance:

  1. Scan the QR Code provided (access via the camera on a smartphone or tablet) OR go to: https://attendsafe.app/unitedway/june2023

  1. Follow the prompts.
  2. Enter the information requested (including COVID vaccination card/records).
  3. Select “upload the image” of your COVID-19 vaccine card/documents.
  4. Once your COVID-19 vaccine card/documents are submitted and verified, you will receive an email confirming your verification.

Acceptable Vaccination Documents:

  • Actual photo of your CDC issued vaccination card clearly showing your full name, vaccination type received, date of shot(s) and administering facility/clinician.
  • Medical records (EMR) including your full name, vaccination type received, date of shot(s) and administering facility/clinician.

If additional information is required to complete your verification, the Attend Safe Verifications Team will contact you via the email and/or cell phone number provided during your submission. If you do receive a follow-up request for additional information, please make sure to respond in a timely fashion, so your entry into the event is not delayed upon your arrival. We suggest that you save and/or print and bring with you, the email verification confirmation for entry to the event.
